My current setup is Image

I thought it was a pretty good setup, took me a couple years slowly upgrading it and since the next gen consoles are coming out I would like to be able to keep up at least a little bit. What would you suggest I upgrade to make my games run a bit smoother? I can nearly play Skyrim on Ultra but then have to put The Witcher 2 on Medium-Low settings. My graphics card could use some updating but other than that I'm lost really...

Really the only thing that needs to be upgraded is your GPU. I would also suggest upgrading to 8GB of RAM and put in an SSD but those aren't absolutely required. What do you have in mind for a budget because that's going to affect what I recommend.

Thanks for the reply. My budget is in the range of $300.

You could get a GTX 660 Ti or Radeon HD 7950 (or 7870, I forget what the price bracket is). Should give you a nice boost.

There are a couple ways to go with this upgrade. Upgrade option one would be purely an upgrade on the video card to a Radeon 7950 as that is by far the best bang for the buck currently. Another option would be to do a bit more thorough upgrade although you might go slightly overbudget. It would consist of:

Gigabyte Radeon 7850 2GB
Samsung 840 120GB
G.Skill RipJaws 4GB DDR3-1600

Unless you can switch to a 64 bit version of windows, you will be stuck with the amount of memory you currently have. The video cards suggested are your best bet.
