I want to set up a 2nd monitor in order to have a dual monitor setup. I currently have an ATI Radeon HD 4850 video card. Based on this card and a two monitor setup, would I have any issues playing games such as CoD, Diablo3, etc? Perhaps not at max settings but is there another way or something I should do in order to get a better setup going?

Thanks in advance.

No, the only caveat is that the other monitor may blank out in full screen games. So if you want to monitor something, you'll have to run in windowed mode.

In terms of gaming performance, will it suffer because it has to send a signal to two monitors?

Partly, as the video card doesn't cut the signal off to the second monitor. So you use some VRAM for the framebuffer (although it may be able to store that in RAM)
