For the Start Menu's "Recent Documents" what you can do is:

1) Right-click on the "Start" button and choose "Properties"
2) In the "Start Menu" tab click on "Customize"
3) Go to the "Advanced" tab and uncheck the "List my most recently opened documents"

This will remove that from the Start Menu. However all the programs that have this feature will still list recent documents so you'll have to find out how to do this for each program that you don't want recent files listed.

If you want to do this with Media Player do the following:

1) Go to "Tools" and choose "Options"
2) Go to the "Privacy" tab
3) In the "History" section uncheck the "Save file and URL history in Player"
