I've been seeing this alot around the web lately on pretty modern sites, could it be a configuration issue on my end?

One example for those of you who use the DownloadHelper FF extension look at this videos download link: http://www.youtube.com/watch?v=_CYwNWHZuT0

It has it where quotes should be. I thought this problem was back in HTML 3 days.... why am I seeing it around the web with a modern browser? This isn't relegated to just the download helper plugin btw, I've seen it in plenty of random blogs etc, sorry I cant give more examples.
